Job Summary:

The Case Manager helps clients navigate services and resources to improve their well-being. Some responsibilities include assessing client needs developing care plans coordinating services and monitoring progress. The Case Manager works closely with healthcare providers social workers and community organizations to ensure clients receive appropriate support. Strong communication problem-solving and organizational skills are essential for success in this role.

Key Responsibilities:

Scheduling and Client Appointments

  • Make every effort to schedule clients for case management appointments.
  • Update and maintain appointment availability.
  • Contact and reschedule all client appointments when availability changes.
  • Answer the phone and door and accept walk-in clients as time and availability permit reschedule as needed.

Client Interaction and Communication

  • Provide comprehensive case management services to all clients.
  • Answer phone calls while at the desk and respond to phone messages within 24 48 hours.
  • Respond to all emails within 2 hours of receipt while in the office or first thing the next day if out.

Documentation and Reporting

  • Document all interactions intakes phone calls referrals mail checks etc. into In Sync
  • Make every effort to document notes in real-time when possible while seeing clients at your desk.
  • All client documentation must be completed within their assigned 3 day window.
  • Develop implement track and update Housing Plans for all HPSJ/Health Net clients.
  • Submit pre-authorizations for Housing Deposits once lease agreements are received by the client.
  • Provide the Director of Case Management with lease agreements to track follow up and bill for services.
  • Source programs services agencies and community supports to build clients support network.
  • CC the Director and CEO on all First Five submittals and denials for tracking purposes.
  • Assist all recuperative care referrals through Sutter as directed. This may include but is not limited to: arrange and pay for rooms provide meals groceries clothing and hygiene items complete intake paperwork provide resources link clients to appropriate medical and social referrals schedule follow-up appointments
  • Attend all trainings calls and other meetings as directed in a timely and professional manner. Be prepared to share updates feedback and review new resources.

Work Environment:

  • This position requires working in an office setting with occasional travel
  • Interaction with clients experiencing housing instability and other challenges is frequent.

Working Conditions

  • None: The worker is not substantially exposed to adverse environmental conditions (as in typical office or administrative work).

Requirements

Qualifications & Skills:

  • Experience in case management or social services.
  • Knowledge of relevant laws and regulations
  • Proficiency in case management software and office tools.
  • Communication & Advocacy Engaging with clients and service providers while ensuring their needs are met.
  • Problem-Solving & Critical Thinking Navigating complex cases and finding solutions.
  • Organizational & Time Management Handling multiple cases efficiently.
  • Empathy & Relationship Building Establishing trust and rapport with clients.
  • Documentation & Attention to Detail Maintaining accurate records and reports.


Physical Requirements


Sedentary work: Exerting up to 10 pounds of force occasionally and/or a negligible amount of force frequently or constantly to lift carry push pull or otherwise move objects including the human body. Sedentary work involves sitting most of the time. Jobs are sedentary if walking and standing are required only occasionally and all other sedentary criteria are met.

Talking: Expressing or exchanging ideas by means of the spoken word; those activities where detailed or important spoken instructions must be conveyed to other workers accurately loudly or quickly

Hearing: Perceiving the nature of sounds at normal speaking levels with or without correction and having the ability to receive detailed information through oral communication and making fine discriminations in sound.
