Job Summary
Partial list of job responsibilities:
- Conduct scheduled in-person meetings with clients in their homes
- Organize medications and assess ongoing medical needs
- Develop medication regimen and checklist
- Dispense correct dosage of each medication at the correct time
- Fill pill boxes or medication dispensers update with new medication orders
- Reconcile medications and notify clients when refills are needed
- Coordinating with healthcare providers regarding changes in medication reorder medications as needed and procure refills
- Check labels expiration dates and warn patients or caregivers about potential interactions
- Observe and document ongoing changes of condition (remotely viewable by family members)
Private duty nurses serve as a liaison between the client family members and healthcare providers to ensure seamless communication and the best possible care plan for the client.
