Job Summary

Corewell Health Geriatrics Division Seeks a Board-Certified Nurse Practitioner or Physician Assistant to join their team providing care to patients residing in Skilled Nursing and Long Term Care facilities located in the northern region of West Michigan. This provider will spend dedicated days at each of the facilities located in Greenville and Reed City area. Excellent physician support and great relationships developed with the facilities. Minimal Call/Weekend Coverage. The ideal candidate will have experience working with this population of patients and enjoy autonomy once fully oriented to the position.

Essential Functions

  • Assesses and monitors the medical needs of patients/significant others.
  • Provides quality medical services to patients in a cost effective appropriate manner and ensures coverage of case load at all times.
  • Provides input into the development of policies and procedures as they related to medical practice and quality patient care.
  • Documents in a manner consistent with regulatory standards.
  • Communicates with other members of the health care team in order to ensure continuity of care and coordination of services. Acts as a patient/family advocate.

Qualifications:

Nurse Practitioner:

  • Required Masters Degree Nurse Practitioners who obtained their education and certification after 2000 must show evidence of completion of a masters post-masters or doctorate from a Nurse Practitioner program that is accredited by the Commission on the Collegiate of Nursing Education or the National League for Nursing Accrediting Commission
  • 3 years of relevant experience current relevant clinical experience
  • Previous experience functioning in a collaborative role as a Nurse Practitioner
  • LIC-Nurse Practitioner (NP) - State of Michigan
  • CRT-Basic Life Support (BLS)

Physician Assistant:

  • Required Masters Degree Graduate of an accredited Physician Assistant educational program
  • LIC-Physician Assistant - State of Michigan
  • CRT-Physician Asst Certified (PA-C) - NCCPA National Commission on Certification of Physician Assistants
  • CRT-Basic Life Support (BLS)
