Job Summary
Home Health Certified Nursing Assistant (CNA)
Summary:
Provides in-home personal care and support to clients under the supervision with the collaboration of the Care Team Manager and Client Care Manager Assists with daily living needs monitors basic health indicators and helps maintain a safe comfortable home environment.
Responsibilities:
Assist with bathing dressing grooming toileting and feeding
Support mobility: transfers ambulation and exercises
Take and record vital signs and report changes in condition
Provide companionship and emotional support
Light housekeeping and simple meal preparation
Qualifications:
Active state CNA certification REQUIRED
1 year experience
Reliable transportation
Strong communication and companionship
