Occupational Therapist (OT) Home Health

Position Summary:
The Occupational Therapist provides home health services focused on delivering patient-centered therapeutic interventions to promote independence function and safety within the home environment. This role requires strong clinical judgment excellent communication skills and the ability to manage a flexible field-based schedule while collaborating with an interdisciplinary care team.
Key Responsibilities:
Provide home-based occupational therapy evaluations and treatments according to patient care plans
Develop individualized treatment plans to improve functional ability mobility and safety
Educate patients and caregivers on exercises adaptive equipment and home safety practices
Collaborate with nurses therapists and care team members to coordinate patient care
Document all care accurately using EMR systems
Travel to patient homes throughout Indiana and surrounding areas
Meet required charting deadlines and maintain compliance with regulatory standards
Participate in scheduled weekend and holiday rotations
Required Qualifications & Skills:
Bachelors or Masters degree in Occupational Therapy from an accredited program
Active Pennsylvania Occupational Therapist license
Minimum 1 year of healthcare experience
Valid drivers license with daily travel required
Proficiency using tablets smartphones and EMR software
Strong communication organization and time management skills
Ability to work independently in a home health environment
Schedule:
Monday Friday 8:00 AM 5:00 PM
One weekend per month
Two holidays per year
