The Registered Nurse will deliver skilled nursing care case management and hospice care to individuals facing advancing age frailty and serious illness. Responsibilities include patient assessments medication administration treatment planning and interdisciplinary team collaboration. Strong communication and collaboration with patients providers and team members are essential.

Responsibilities:
- Conduct patient assessments to determine healthcare needs.
- Administer medications as prescribed by physicians.
- Develop and implement patient treatment plans.
- Collaborate with the interdisciplinary team to coordinate patient care.
- Provide skilled nursing care to patients.
- Manage patient cases to ensure optimal outcomes.
- Offer hospice care and support to patients and their families.
- Maintain accurate and up-to-date patient records.
- Communicate effectively with patients providers and other team members.
Required Skills & Certifications:
- Active Registered Nurse license in the state of working.
- Bachelors degree in Nursing or related field.
- Clinical Assessment skills.
- Medication Administration skills.
- Treatment Planning skills.
- Interdisciplinary Team Collaboration skills.
- Minimum of 2 years of hospital home health or hospice care experience.
- Drivers license car insurance and automobile for field work.

- Shifts between 7:00 AM and 10:00 AM.
- Approximately every other to every third weekend from 8:00 AM to 5:00 PM.

- Must work the day after Thanksgiving and the days leading up to and after Christmas/New Year.
