Job Summary

Position:Lead Person-Centered Care & Programs
Department:Programs & Advocacy
Reports to: Vice President Programs & Advocacy
Location:Hybrid (two days in the office per week). The Lung Health Foundation isbased in Toronto and operates in a hybrid model in which staff are required towork at the office a minimum of two days each week with occasional travel within Ontario and across Canada as required.
Salary range:$65000 - $75000
Notes:This is a temporary 16-month (approx.) parental-leave replacement contract position pending the return of the incumbent. This role does not include employee benefits; however the selected candidate may choose between receiving 6% vacation pay or 15 days of paid vacation.

POSITION OVERVIEW
The Lead Person-Centered Care & Programs will be responsible for supporting Lung HealthFoundations key projects and programs for Canadians with lived experiencesworking collaboratively across the Programs Department and organization. This rolecontributes to the execution of program services and support for Canadians with livedexperiences using evidence-based approaches. The lead will actively collaborate and supportthe shared deliverables within the Programs Department engaging with multiple stakeholderswithin and external to the organization. Under the guidance of the Director the Lead will helpsupport engagement implementation evaluation and dissemination of program services toimprove future offerings.
Core to the Leads success will be the ability to establish and foster relationships with diversepartner groups with an emphasis on people living with lung disease. By building strongpartnerships and driving innovation the Lead will help shape the future of respiratory careacross Ontario and Canada. The ideal candidate brings a strong background in respiratoryhealth a commitment to health equity and a passion for improving the lives of patients andcommunities.
Key areas of focus include:
  • Engaging and directly supporting community partners including people with lived experiences caregivers and advocates.
  • Assisting in stakeholder engagement to support programmatic evolution including policymakers clinicians and people with lived experiences.
  • Supporting the design implementation and dissemination of strategic initiatives and program services in alignment with evidence-informed approaches.
  • Assisting in quality improvement efforts to improve current and emerging client care and service delivery.
The Lead will collaborate with teams across LHF to align project efforts with the needs ofcommunity partners organizational goals and contribute to emerging initiatives and prioritiesas they evolve.
RESPONSIBILITES
Program and Network support

The Lead will support the design and implementation of strategic initiatives primarily focusedon advancing the services and support for the Loop Network andcommunity partners. Key responsibilities include:
  • Building relationships with community members by fostering a welcoming supportive online environment for individuals affected by lung disease their families and caregivers.
  • Facilitating virtual support groups discussion forums and peer-to-peer engagement encouraging active participation.
  • Cultivating relationships with community organizations and individuals with the objective of building mutually beneficial partnerships.
  • Coordinating activities which include set-up administration tracking and monitoring of participant registration and attendance program schedules and evaluation instruments.
  • Representing the department at meetings and community events.
Project and Evaluation Support
  • Assisting individuals affected by lung disease to inform the development and improvement of community-focused initiatives.
  • Collaborating with the LHF team to devise evidence-informed approaches to design programs/ services and to collect data for ongoing program services and to support organizational initiatives e.g. online surveys interviews group meetings.
  • Collaborating with cross functional teams to manage social media and website updates to promote and disseminate resources to the larger community.
  • Supporting the evaluation process of programs and services.

Lung Health Foundation Champion
The Lead will:

  • Support and contribute to additional programs or initiatives based on current and emerging priorities and needs;
  • Help grow and maintain LHFs network of community partners and people with lived experiences (e.g. living with lung conditions nicotine experience caregivers)
  • Support cross-functional duties aligned with organizational goals
  • Assist in external communications and advocacy efforts including media engagement in collaboration with the Marketing and Communications team
QUALIFICATIONS

  • University Degree or graduate degree in public health health sciences life sciences or a related field; graduate-level training is considered an asset.
  • Experience working in respiratory health primary care or health systems considered a strong asset.
  • Minimum three (3) years of experience in:
  • Supporting people with lived experience (i.e. patients caregivers public)
  • Health promotion program implementation chronic disease management
  • Stakeholder engagement and co-design in programs and services
  • Strong understanding of evidence-based practice quality standards and patient-centered care.
  • Demonstrated experience with project coordination community development outreach and partner relations.
  • Fluency in French is considered an asset.
