Job Description:

Perform initial evaluations and create individualized treatment plans
Provide daily therapeutic interventions to improve mobility strength balance and function
Document evaluations treatments and progress notes accurately and on time
Collaborate with nursing OT SLP and IDT for coordinated patient care
Educate patients families and staff on mobility safety and exercise programs
Recommend and adjust assistive devices and DME
Ensure patient safety and implement fall-prevention strategies
Participate in care plan meetings and discharge planning
Monitor patient progress and update goals or treatment plans as needed

Certifications:

BLS (AHA)
PT License/Cert
