Job Description:
We are seeking a dedicated Physical Therapist to provide high-quality rehabilitative care in a skilled nursing facility setting. The PT will conduct evaluations develop individualized treatment plans and deliver daily therapeutic interventions to support functional improvement mobility and overall patient safety.
Responsibilities:
Perform comprehensive initial evaluations and develop individualized treatment plans.
Deliver daily therapeutic interventions focused on mobility strength balance and functional independence.
Maintain accurate and timely documentation of evaluations treatments and progress notes.
Collaborate closely with nursing staff occupational therapy speech therapy and the interdisciplinary team.
Educate patients families and staff on mobility techniques safety strategies and exercise programs.
Recommend fit and adjust assistive devices and durable medical equipment as needed.
Implement fall-prevention strategies and ensure patient safety at all times.
Participate in care plan discussions interdisciplinary rounds and discharge planning.
Continually monitor patient progress and revise goals or treatment plans based on clinical needs.
