Family Outreach Services Success Coach
Families and Youth Innovations Plus (FYI)
Position: Full-Time; Hourly/Non-Exempt
Hourly Rate: $17.98 - $21.76 (3 levels based on experience and education)
Reports to: Family Outreach Program Supervisor
Department: Family Outreach Services
Job Summary
The Family Outreach Services Success Coach role ensures timely response and acceptance of referrals from New Mexico Statewide Central Intake (SCI). The Success Coach serves families who are reported and screened out for investigation for child abuse and neglect. The emphasis of case management is on assessing and addressing the family needs to better support any future prevention of child abuse and neglect.

What youll Do:
- Provide clients with supportive services
- Coordinate services between providers and clients
- Interpret agency services to clients and community
- Provide case management services and skill-building services to youth in the program
- Maintain individual case records on clients receiving services
- Provide specialized case management services to individual client needs
- Educate clients on available community resources and services
- Create programs and services to benefit clients and the community
- Attend community meetings relevant to program and clients
What youll Need:
- Bachelors degree in social work or related human service fields.
- At least 1 year of internship or post graduation experience OR two years of combined experience working with communities on health or social service related matters case manager experience
- Excellent communication skills both written and verbal
- Exhibit skills in comprehending interpreting and completing delegated tasks efficiently
