Community Health Connections (CHC) founded on February 25 2002 is a mainstay for compassionate care regardless of income or insurance status. We have over 20 years of experience asa Federally Qualified Health Center (FQHC) serving everyone who comes through our doors; including low income underinsured uninsured publicly housed and homeless populations in 35 cities and towns in North Central Massachusetts.

Under the supervision of the OBOT Nursing Supervisor the Licensed Drug and Alcohol Counselor (LDAC/CDAC) provides a wide range of psychosocial evaluation/assessment diagnostic counseling therapy crisis intervention and/or case management services in an integrated primary care setting located in a community health center. Works collaboratively with other mental health and medical providers and trainees engaged in related therapeutic/patient care activities.

Responsibilities include:

  • Conduct intake assessments to evaluate individuals appropriateness for our program.
  • Use clinical tools (urine drug screens treatment plans counseling etc.) to monitor clinical progress and promote and support recovery.
  • Lead weekly group therapy sessions that build recovery skills and foster community by utilizing the outlined curriculum.
  • Conduct weekly individual one on one sessions.
  • Provide individual support as needed such as crisis intervention referrals to other resources and liaising with outside parties involved in the clients treatment.
  • Measure the effectiveness of ones own clinical practice using outcome measures and client feedback.
  • Practice within state federal and provincial laws/regulations and professional ethical standards including following HIPAA and (42 CFR pt 2) CRF regulations.
  • Actively work to ensure cultural diversity is preserved and upheld.
  • Utilize EMR to thoroughly document clients progress and clinician interactions in a manner that is efficient and value-added fully digitized and never bureaucratic.
  • Participate in discharge decisions in coordination with the clinical supervisor based on member progress in treatment.
  • Engage in constructive and effective clinical supervision.
  • Participate in internal and external training required for upkeep of licensure and actively working towards professional development.

Minimum Qualifications:

  • Bachelors Degree required.
  • Required licensure or certification*: Behavioral health licensure including but not limited to: LCSW LPP LMHC LPCC LPCC-S LPC LCADC CADC LDAC
  • Strong interpersonal and communication skills and the ability to work effectively with a wide range of constituencies in a diverse community.
  • Knowledge of community mental health resources and willingness to reach out to existing resources.
  • Ability to maintain emotional stability to cope with human suffering emergencies and other stresses.
  • Bilingual/Trilingual (ENG/SPANISH/PORTUGUESE) is a plus
  • Knowledge of crisis intervention techniques.
  • Ability to evaluate the progress of therapeutic programs and to make individual modifications.
  • Ability to create compose and edit written materials.
  • Knowledge of the principles and methods of social and vocational rehabilitation as they relate to mental health clinical practices.
  • Knowledge of legal and ethical issues related to patients rights.
  • Ability to lead and train staff and/or students.
