Responsibilities
Works cooperatively with clinicians administrators and medical staff to design implement and monitor an organizational medication safety program that meets accreditations standards as well as federal and state regulatory requirements. Reviews all reports of medication related events and coordinates associated process improvement initiatives and action plans. Promotes a culture of safety within the Pharmacy Service Line.
Qualifications
EXPERIENCE AND EDUCATION:
MINIMUM REQUIRED: Bachelors Degree in Pharmacy and five (5) years of experience in operational and clinical areas of pharmacy operations.
PREFERRED: Doctorate of Pharmacy Degree (Pharm D). Completion of ASHP residency. Expertise in information technology and acute care pharmacy practice.
LICENSES/CERTIFICATIONS
MINIMUM REQUIRED: Valid State of California Pharmacist license.
