Empath Home Health a part of Empath Health is growing! Were expanding our home health services and looking for a compassionate full-timeHome Health LPN to join our team in north Pinellas & west Pasco counties.

This is an exciting opportunity to serve patients in their homes with the flexibility to set your own schedule and grow with us as we continue to build our presence in the this full-time role youll deliver individualized care build trusted patient relationships and help people manage their health with dignity and independence.

Locations: Home health visits in north Pinellas county and west Pasco county (Clearwater/Largo up to Hudson/New Port Richey)

Schedule: M-F 40 hours; weekend and holiday on-call rotations required.

What Youll Do

• Provides nursing services in compliance with agency policy and procedure state and federal regulations and the patients plan of care under the direction and supervision of a Registered Nurse.
• Observes patients and report adverse reactions to medication or treatment to the Clinical Coordinator.
• Assists the Physician and/or Registered Nurse in carrying out the plan of care and in performing specialized procedures.
• Performs patient and family instruction on diet medication and treatment.
• Collaborate with the interdisciplinary team to ensure seamless holistic care

What Youll Need

• Active Florida LPN license
• Applicants must be a graduate of an approved nursing program.
• Minimum of 1-2 years of experience as a LPN.
• Home health care experience preferred.
• Strongly prefers experience using Homecare Homebase (HCHB) EMR
• Intravenous (IV) Certification strongly preferred.
