Direct Patient Care

Must be certified via Prometric Hawaii. Must be able to work all weekends 40hours per week Rotating shifts.

Assist with ADLs including:

• Bathing

• Dressing

• Feeding

• Toileting

• Ambulating and mobility support

• Skin care and hygiene

• Take and record vital signs weights and intake/output accurately.

• Report patient condition changes immediately to the Charge Nurse.

Support & Observation

• Observe residents for signs of discomfort skin breakdown or behavioural changes.

• Provide comfort measures such as repositioning hydration and emotional reassurance.

• Assist with admissions discharges and transfers.

Safety & Compliance

• Always Follow infection control and Universal Precautions.

• Maintain a clean and safe environment.

• Protect patient confidentiality and resident rights.

Communication & Documentation

• Document care activities in accordance with facility standards.

• Communicate effectively with nurses physicians residents and families.

• Participate in hand-off reporting and care team meetings.

Teamwork & Professionalism

• Work collaboratively with nursing and support staff.

• Demonstrate compassion patience and professionalism in interactions.

• Adapt to changing care needs and maintain a positive attitude.
