Requirements: NY LPN License BLS (AHA or RQI only) 1 year LPN experience holiday rotation required.
Complete Description (Condensed):
The LPN provides direct and indirect skilled nursing care to adult and geriatric residents under RN supervision. Responsibilities include ongoing assessment data collection contributing to individualized care plans and supporting safe age-appropriate culturally relevant care. The LPN evaluates resident responses recommends plan-of-care revisions and collaborates with residents families nursing staff physicians and interdisciplinary team members. This role requires maintaining clinical competency participating in staff orientation/mentoring and following all LTC policies and infection-control standards. Potential exposure to blood-borne pathogens may occur.
Key Duties:
Collect and report physical/psychosocial assessment data to RN/IDT
Assist in developing and updating resident care plans
Recommend care-plan revisions based on resident outcomes
Maintain safety efficiency and adherence to LTC policies
Participate in continuing education to maintain competency
Support orientation/mentoring of staff as directed
Promote effective communication with residents and families
Required Skills/Experience:
