    Role: Contract Full-Time: Until May 29 2026. 35 hours per week evenings and weekends may be required.

    Closing date: December 13 2025

    Salary: Starting salary range $ 56444-$60461 *This represents the first three steps in a seven-step salary grid. Candidate experience will be considered when determining the starting salary.

    Location: The successful candidate will work primarily from our Markham Office located at 3601 Highway 7 East HSBC Tower Suite 710 Markham ON L3R 0M3 CAN. Travel within York Region & South Simcoe and to other CMHA YRSS locations will be required.

    Job Profile:

    The Case Manager works with clients 16 years of age and older who are experiencing significant difficulties in day-to-day and role functioning as a result of serious and persistent mental illness and/or addictions. The Case Manager collaborates with the client to identify strengths and needs and develop long term goals short term objectives to meet these goals and concrete action steps to achieve the objectives. As part of this process individuals are assisted to acquire and use skills and to connect with formal and informal supports peer support professional services and resources to promote their recovery improve their quality of life and reduce the likelihood of relapse. Other duties as required.

    The Case Manager collaborates with clients to provide individualized and flexible services to engage support and integrate clients into the community and facilitate their use of resources and supports to maximize their independence and potential. The Case Manager adheres to agency Health & Safety Program and the Client Safety Framework in the promotion of client safety. The Case Manager participates in individual and systemic advocacy initiatives to improve resource access and availability.


    In addition to compliance with all organizational policies and procedures the incumbent is also expected to behave ethically demonstrate competence in effective communication and team building demonstrate effectiveness in working in a collaborative environment and demonstrate confidence in decision-making and in building relationships.

    Requirements:

    • A degree or diploma in the health or social services. Preference is given to a degree in a related discipline.
    • One or more years of experience in a community-based organization working with adults with serious mental illness in the community.
    • Full registration and in good standing with the OCSWSSW or a regulatory body under the Regulated Health Professions Act
    • Experience working with individuals with Addictions is an asset
    • Second language (Cantonese Mandarin Farsi or Tamil Italian Korean Punjabi Russian Urdu) reflecting the local community is an asset.
    • Knowledge of the Ontario Mental Health Act mental health reform principles the Substitute Decisions Act the Health Care Consent Act and PHIPPA requirements.
    • Knowledge of PSR and Recovery as guiding principles.
    • Ability to apply Recovery principles and empowerment-oriented philosophies and practices in work with clients.
    • Extensive knowledge of supports and services in York Region and South Simcoe including formal and informal resources.
    • Knowledge of systemic issues such as poverty unemployment stigma medications and the isolation felt by individuals with serious mental health issues and their families.
    • A valid Canadian Full G drivers license a minimum $third party liability insurance and proof that insurance covers use of personal vehicle to transport clients
    • A satisfactory Vulnerable Sector Screening (Police Check)
