Job Summary

Description

Responsible for carrying out clinical pharmacy services promoting clinically and evidence-based rational drug therapy and sound pharmaceutical care through the development of new (or expansion of existing) pharmacy practice programs drug therapy policies and other programs. Serves as a clinical pharmacy resource for the health system and its affiliated pharmacy services. Responsible to serve as a preceptor for the residency program and for students from local colleges of pharmacy. Conducts research and other scholarly activities throughout the year with the goal of publishing. Supervises and guides pharmacists on the optimal provision of care to ensure professional development and through use of their independent judgment directs and disciplines their employees. Other responsibilities include patient care rounds clinical interventions medication use evaluations and institutional committee membership.



Responsibilities

1. Demonstrates the ability to perform clinical/technical/service/administrative tasks:

Conducts and participates in departmental activities to meet the goals objectives and policies of the department hospital/medical center by the standards set forth by Federal and State Regulatory agencies and The Joint Commission

Develops and implements policies and procedures to meet professional standards and improve medications safety including those established by the American Society of Health-System Pharmacists (ASHP)

Implements decisions of and provides support for the Pharmacy & Therapeutics committee by preparing drug evaluations and assisting in formulary maintenance

Uses an interdisciplinary approach to collaborate with other health care professionals to plan monitor review and evaluate the cost utilization safety and effectiveness of drugs and drug regimens and the elimination of unnecessary duplication of service in order to provide optimization of medication therapy

Attends interdisciplinary rounds and provides and follows up on recommended interventions with prescribers interdisciplinary care team other pharmacists patients and/or caregivers including appropriate documentation of recommendations and interventions

Engages in active patient-centered medication therapy management including medication reconciliation services and discharge services patient counseling on appropriate medical therapy conducting comprehensive medication reviews and targeted interventions to assess appropriateness of all medications in order to improve clinical outcomes reduce unnecessary readmission lapses in care and adverse drug reactions.

Maintains patient/employee confidentiality in the management of information

Coordinates the provision of pharmaceutical care by pharmacy department members

Ensures the work environment is well maintained

Demonstrates initiative and leadership

2. Remains current with various pharmaceutical databases national guidelines and follows evidence-based medicine for medication therapy management of disease states

3. Provides education to patients family members and/or caretakers about disease states and medications including:

Desired results and/or goals of chronic disease states

Name strength description and purpose of each medication

How to administer each medication timely safely and appropriately

How agents work to attain adequate therapy

Interacting foods disease states and drugs where appropriate

Importance of therapeutic drug monitoring for efficacy and toxicity

Recognition of disease state exacerbation and procedures to follow in these instances

Recognition of side effects of therapy and the importance of notifying his/her physician if any should occur

Importance of adherence to pharmacotherapy and appointments

Aids to increase medication adherence

4. Demonstrates the knowledge and skills necessary to provide care based on physical psychosocial educational safety and related criteria appropriate to the age of the patients served in assigned area. (check all populations served)

Neonate/Infant Pediatric Adolescent Adult Geriatric

Identifies physical behavioral and emotional characteristics typical for the age group

Modifies approaches based on patient age-specific needs and responses to treatment

Provides care for patients based on age-specific needs

Uses communication techniques which are age appropriate

5. Educational/Professional Development

Participates in the development of other staff members by identifying the learning needs of pharmacy staff and other healthcare professionals through education on but not limited to: clinical appropriateness of medications dose adjustments drug selection based on formulary drug interactions drug cost and patient safety

Serves as a preceptor for the residency program and for students from local colleges of pharmacy

Designs and implements appropriate trainings and development programs in a timely fashion to maintain contemporary practice

6. Supervisory Functions

Supervises pharmacy employees

Interviews candidates for pharmacy management and staff positions

Implements discipline and corrective action plans for assigned pharmacy staff

Promotion of employees based on assessment of clinical skills if applicable

Responsible for employee performance evaluations for assigned staff

Attends and contributes to pharmacy management meetings

Provides off-hour coverage for the department



Qualifications

Education Requirements

Doctor of Pharmacy Degree required


ASHP-Accredited PGY-1
required;PGY-2 preferred


Board Certification (e.g.BCPS BCCCP) strongly encouraged

Experience Requirements

3 years hospital experience or equivalent with clinical experience/training

Licensing and Certification Requirements

Name: Registered Pharmacist Issuing Agency: NYS Board of Pharmacy

Continues to meet regulatory educational and licensure requirements throughout employment as set forth by New York State regulations.

Achieves annual requirement of 15 continuing education hours (ACPE approved).
