Your role and responsibilities

You will:

  • clinically review referrals into the Healthy Living Service and take appropriate actions

  • undertake clinical and wellbeing supervisions for the Healthy Living Practitioners

  • support with the daily operational components of the Healthy Living Service

  • work in accordance with the NMC Code of Professional Conduct and other regulatory documents ensuring your NMC registration is maintained

  • support the needs of children young people and their families through ensuring the implementation of the care pathways outlined in the Healthy Living Service Standard Operating Procedures (SOP) and in accordance with clinical governance frameworks and policies.

You will need:

  • to be a current Nursing and Midwifery Council (NMC) Registered Nurse
  • ability to provide daily oversight and supervision management of people and services
  • knowledge of Quality Assurance and Performance Management
  • skills to review records of children with unhealthy weight take appropriate actions and identify early interventions to reduce risks and improve future health and wellbeing
  • a thorough understanding of information and advice to signpost children young people and families to relevant universal and specialist services locally and beyond empowering families to help themselves
  • ability to safeguard children young people and vulnerable adults through effective practice and multi-agency working including participation in safeguarding meetings and relevant assessments
  • capacity to support children young people and their families to maintain good emotional health and wellbeing helping them find support networks from friends family and the voluntary community faith and social enterprise sector.

The Healthy Living Service is part of the Community Health Team within the Children and Young Peoples Service. Our team brings together dedicated professionals from health social care and childrens centres working collaboratively to support families during the most crucial years of a childs life. We share a commitment to giving every child in Suffolk the best possible start tackling challenges together and promoting lifelong health and wellbeing. By combining expertise and a multi-agency approach we provide tailored advice assessments and interventions that make a real difference to families.
