Duties

We are seeking a passionate and skilled professional to join our Assertive Community Treatment (ACT) team in a unique dual role. Our ACT team is a dynamic multidisciplinary group dedicated to providing comprehensive care to adults with severe and persistent mental illness in our community. You will be instrumental in empowering clients from underserved communities by blending case management with vocational rehabilitation. As a Vocational Specialist you will engage and support clients to identify their vocational and educational goals; conduct vocational assessments to understand skills interests and strengths; spearhead job development by building relationships with employers and educational institutions; support clients through job placement and school enrollment provide job coaching and follow-along supports in workplace and academic settings to ensure long-term success and offer benefits counseling to help clients understand how employment impacts their benefits.

As a Case Manager you will serve as a central point of coordination working closely with the ACT team community agencies and families; connect clients with vital resources and coordinate all aspects of their treatment process ensure clients maintain contact with their psychiatrist and follow up to support medication adherence complete comprehensive functional assessments to inform personalized care plans build strong therapeutic relationships with individual clients and their families develop partnerships with other community service providers to create a robust network of support.

Job Qualifications

Bachelors degree in a Behavioral Sciences or Related field required. Experience may not be exchanged for education. Minimum 1 year experience in family outreach support counseling or community mental health. Minimum 1 year of experience providing employment services or advanced education that involved field training in vocational services to individuals with mental illness (e.g. job development job coaching supported employment). Must meet mental health practitioner requirements of law. Valid Drivers License auto insurance and reliable vehicle required.
