Job Summary
Job Title: Registered Nurse Long-Term Care (LTC)
Location: Rifle CO
Type: Contract
Pay Rate: From $43/hr
Position Overview:
Navitas Healthcare LLC is seeking Registered Nurse Long-Term Carefor an exciting Travel or Local job in Rifle CO.
Responsibilities:
Provide direct nursing care to residents including assessments medication administration and treatment plans.
Monitor and document vital signs medical conditions and changes in resident health.
Supervise and guide nursing assistants and support staff to ensure quality care.
Participate in care plan development updating and implementation for each resident.
Maintain accurate and timely documentation in compliance with facility policies and regulatory standards.
Educate residents and families on care plans health management and procedures.
Assist with resident transfers mobility and activities of daily living (ADLs).
Support infection control practices safety standards and quality improvement initiatives.
Qualifications:
Active RN license in the state of Colorado.
Minimum 2 years of experience in Long-Term Care (LTC) or Skilled Nursing Facility (SNF).
Strong clinical assessment and critical thinking skills.
Excellent communication and teamwork abilities.
Commitment to patient/resident safety dignity and quality care
