The Physical Therapist provides home health physical therapy services to patients in Salem OR and surrounding areas. This role involves assessing patient needs developing and implementing treatment plans and working to improve patients mobility and overall well-being. The therapist will manage a caseload of patients ensuring appropriate documentation and communication with the healthcare team.

Responsibilities:
- Conduct patient assessments to determine physical therapy needs.
- Develop and implement individualized treatment plans.
- Provide skilled physical therapy interventions to improve mobility and function.
- Document patient progress and communicate with the healthcare team.
- Manage a caseload of home health patients optimizing visit schedules.
- Ensure accurate and timely completion of Oasis documentation.
- Maintain compliance with facility policies and procedures.

Required Skills & Certifications:
- Active Physical Therapist license in the state of Oregon.
- Basic Life Support (BCLS) certification.
- Minimum of 1 year of skilled home health experience.
- Valid drivers license and reliable transportation.
- Proof of car insurance.
- COVID-19 vaccination primary series and one booster.

Preferred Skills & Certifications:
- Experience with EPIC Remote Client for Home Health and Rover (phone app).
- Experience with EPIC Remote Client for Home Health and Rover (phone app).
Special Considerations:
- Traveler must have their own transportation.
- The facility can call off up to one shift per pay period (every two weeks).
- Mileage reimbursement follows IRS standards.
- Business casual attire is required; no jeans. Scrubs are allowed but not required (no specific color).
