As a Physician Assistant or Nurse Practitioner youll join a purpose-driven team that:
- Delivers continuum-based care integrating primary psych palliative and home-based services.

- Continuum-Based Care (Clinical Excellence): Deliver high-quality care across home senior living and community settings support by a multidisciplinary team in palliative psych and neurology.
- Dementia Leadership: Build deep expertise in dementia-informed care and caregiver support as part of our expanding geri-psych and cognitive health initiative.

Your responsibilities include:
- Delivering personalized high-quality care to seniors in their homes assisted living hospice and nursing facilities.
- Delivering dementia-informed relationship-based care across the continuum of care.
- Conducting comprehensive assessments and developing tailored care plans for older adults.
- Managing chronic conditions and performing care with skill and empathy for the patients and their families.
- Collaborating with cross-functional partners including palliative psych and neuro to deliver coordinated whole-person care.
- Building trusted relationships with residents caregivers and facility partners to enhance engagement and referrals.
- Maintaining consistency in documentation follow-through and patient communication.

- Active unencumbered NP or PA license in the state you will be practicing
