Job Summary:
Navitas Healthcare LLC is seeking Occupational Therapistfor an exciting job in Greenbrae CA.
Key Responsibilities:
Conduct comprehensive evaluations and develop personalized occupational therapy plans
Deliver therapeutic interventions to improve ADLs mobility strength and functional performance
Document evaluations treatments and progress per facility and regulatory standards
Collaborate with interdisciplinary team members to coordinate resident care
Educate residents and families on therapy goals techniques and home exercise programs
Maintain a safe and supportive therapeutic environment
Participate in discharge planning and provide professional recommendations
Qualifications:
Active California Occupational Therapist (OT) license
Minimum of 1 year of clinical experience; SNF experience preferred
Valid BLS certification
Strong clinical assessment communication and documentation skills
Ability to work independently and collaboratively within a rehabilitation team
