Job Title: Physical Therapist Home Visits
Location: Malone NY (Franklin County)
Salary Range: $80000 $100000 per year
Employment Type: Full-Time / Direct Hire

A supportive employee-owned home care organization in Northern New York is seeking a dedicated Physical Therapist to provide in-home care to adult patients. Responsibilities:

Conduct home-based evaluations using diagnostic muscle nerve joint and functional ability assessments.
Develop and implement individualized plans of care based on patient needs and physician orders.
Provide direct therapy services to improve mobility strength and independence.
Educate patients and caregivers on therapy goals home exercises and safe use of mobility aids or assistive equipment.
Maintain accurate and timely clinical documentation for each visit.
Participate in team meetings case conferences and in-service training.
Adhere to safety standards and ensure equipment is properly maintained.

Qualifications:

Graduate of an accredited Physical Therapy program.
Current and valid Physical Therapy license issued by the New York State Department of Education.
Minimum of one year of clinical experience as a licensed physical therapist.
Strong communication assessment and organizational skills.
Ability to work independently and manage a flexible home visit schedule.

Compensation and Benefits:

Annual salary range: $80000 $100000 based on experience.
