Program Description
NY/NY III Scattered Site Housing Program Option II: This program provides 65 units of scattered-site supportive housing to single chronically homeless adults (18 or over) who have a substance use disorder that is the primary barrier to independent living and who also have a disabling clinical condition. Staff provides case management and other supportive services to enable clients to achieve the skills and financial independence necessary for independent living.

What The Case Manager Does:
The Case Manager is responsible for ensuring that clients are connected to needed services to address immediate needs as well as ensuring that clients obtain stability to maintain permanent housing. Maintain clear organized and audit ready records. Report all services provided to clients into program database monthly. This work is in support of the mission and goals of CAMBA.
