Job Summary:
The Registered Nurse provides skilled nursing care case management and hospice care to individuals facing advancing age frailty and serious illness. Responsibilities include patient assessments medication administration treatment planning and collaborating with the interdisciplinary team. Strong communication and collaboration with patients providers and other team members are critical.

Responsibilities:
- Provide skilled nursing care
- Perform patient assessments
- Administer medications
- Develop and implement treatment plans
- Collaborate with the interdisciplinary team
- Communicate effectively with patients providers and team members
- Float between like units including mother baby
- Precept other nurses
Required Skills & Certifications:
- Active Registered Nurse license
- ACLS certification
- BCLS certification
- FHM certification
- NRP certification
- Minimum 3 years of experience
- Clinical Assessment Medication Administration and Treatment Planning skills
- Interdisciplinary Team Collaboration skills
- Excellent communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
- Bachelors degree in Nursing or related field
- Drivers license car insurance and automobile for field work
Preferred Skills & Certifications:
- Precepting skills
- AWHONN Advanced certification
Special Considerations:
- Must be flexible working weekends
- May be required to float to other facilities
- Facility is able to call off once per two week pay period
Scheduling:
- PM shift
- Weekend work may be required depending on contract length
- Block scheduling is per manager discretion
