FL DCF Macclenny NEFSH Social Services Counselor

Pay rate: $30.50-$32.00 per hour.

Location:7487 South State Rd 121 Macclenny FL 32063.

Hours: 8:00am-4:30pm 40 hours per week.

Work environment: This is a State of Florida Psychiatric Hospital which operates 24/7 7 days a state-owned hospital has 613 beds and 19 living areas. They provide inpatient psychiatric care for clients committed to the hospital by the state or through the criminal justice system.

The hospital accepts adults 18 and older whose needs cant be met through treatment in a community-based program. Theprogram provides evaluation assessment and treatment to clients who are deemed not guilty due to insanity or who are incompetent and cant proceed with criminal proceedings.

Driving requirement:Will beavailable to assist other staffwith transportation on recreational outings escortindividuals to appointments forprosthetic devices or hospital wide celebrationsand transport individualsto the community for dischargereadiness appointmentsor actualdischarge.

Duties:

  • Participates actively in the treatment process attending treatment team meeting and contributing to the development and revision of individuals under treatment with their therapeutic plan. Interprets critical information about the individuals social history and current social situation to the team. Records community needs of individuals at the meetings and jointly works with the team in making recommendations for treatment and discharge planning. Complete level of care (LOCUS) Assessments at the following intervals: Baseline Annual Semi-Annual and upon Discharge.
  • Coordinates planned and unplanned discharges from the unit and serves as the point of contract between the community liaison with the catchment area and the unit arranges an after-care plan for unit-based discharges notifies significant other of discharge plans arranges for transportation when necessary. Completes necessary discharge documentation and discharge notes. Develops and maintains expertise regarding community resources to prepare individuals for discharge.
  • Conducts individual counseling and group psychotherapy with chronically mentally ill persons including geriatric individuals. Assesses level of functioning with regard to social and community life skills necessary for discharge to the community.
  • Documents all social services treatment and unit discharge activities in the clinical record. Completes weekly and monthly progress notes psychosocial histories discharge notes for unit-based discharges admission and movement records and event notes as appropriate. Updates the accuracy of face sheets including information on changes of addresses and phone numbers guardians and guardian advocates 1st representative and other significant persons necessary to ensure quality continuity of care.
  • Interviews/contact family members of individuals being served to obtain data related to personal social and emotional history. Maintains regular contact with family or significant other through treatment team meetings telephone contacts and letters in order to provide updates on the individuals condition and current discharge plans.
  • Coordinates day passes and leave of absences with the treatment team legal department and the ARC Committee as required. Coordinates referrals for prosthetic devices and medical equipment as necessary. Assists with habeas corpus requests and court appearances a necessary.
  • Will be available to assist other staff with transportation on recreational outings escort individuals to appointments for prosthetic devices or hospital-wide celebrations and transport individuals to the community for discharge readiness appointments or actual discharge placement. Performs other related duties as assigned and assists in training new Social Services Counselors.

Qualifications:

  • Hold a Valid Florida drivers license and pass a level 2 background.
  • A completed bachelors degree in a human services fieldanda minimum of three years of professional experience working with individuals who are mentally ill or have disabilities.
  • Ability to complete Mandt CPR and First Aid Training.
  • Ability to use a computer and skilled in Microsoft Word Excel Power Point Outlook and Lotus Notes.
  • Knowledge of social work and behavioral interventions and procedures case management and community referrals.
  • Skills in interviewing assessing problem-solving and crisis management.
  • Ability to collect synthesize and interpret behavioral physical and emotional patters and complete clinical documentation.
  • Ability to lift and carry up to 50 pounds and the use of a safety lift belt.
  • Ability to walk extended distances across campus.
  • Physical interventions with combative individuals may be required.11. Physical ability to push pull bend and stoop as needed.
