Job Summary

The Physical Therapist is responsible for delivering professional rehabilitation therapy to patients in their homes. This includes evaluating patient needs developing individualized goal-oriented treatment plans and implementing therapies in accordance with organizational policies clinical standards and regulatory guidelines.

Essential Duties & Responsibilities

  • Complete comprehensive patient evaluations within required timeframes ensuring a holistic assessment of the clients condition.

  • Develop individualized treatment programs with functional measurable short- and long-term goals.

  • Prepare accurate and timely documentation including progress notes reassessments and discharge summaries.

  • Ensure documentation supports the need for continued skilled care and adheres to policy standards.

  • Deliver therapy according to the frequency and duration prescribed in the physicians plan of care.

  • Serve as liaison between physicians and other healthcare team members to coordinate services and resources.

  • Supervise Physical Therapy Assistants (PTAs) evaluate competence and complete required co-signatures.

  • Provide patient and/or caregiver education including a written home program for the medical record.

  • Build rapport with patients to support progress and goal achievement.

  • Meet productivity requirements consistently and independently.

  • Participate in discharge planning throughout the care episode.

  • Maintain accurate and timely scheduling using designated systems.

  • Respond promptly to messages and document interdisciplinary communication.

  • Complete discharge summaries summarizing patient status achieved outcomes and recommendations.

  • Perform additional projects or duties as assigned; demonstrate flexibility across responsibilities.

  • Maintain compliance with organizational standards of behavior and competencies.

  • Ability to lift push and pull a minimum of 50 lbs. and stand for at least 8 hours per day.

Required Skills & Experience

  • Minimum 6 months of Physical Therapy experience.

  • Competence with computer systems and electronic documentation.

  • Strong PT assessment skills clinical intervention capability and multitasking ability.

Preferred Experience

  • Home care experience (Part A) strongly preferred.

  • Knowledge and current use of OASIS required.

  • Experience with EPIC preferred.

Education Requirements

Required

  • Bachelors Degree in Physical Therapy

Preferred

  • Doctorate in Physical Therapy (DPT)

Licensure & Certifications

Required

  • Registered Physical Therapist (active valid license)

  • AHA BLS Certification

Preferred

  • Certified Home Care experience (Part A)

  • OASIS proficiency

Schedule Details

  • Shift: Day Shift (58 hours)

  • Hours per Day: 8

  • Hours per Week: 40

  • Weekend Requirement: Every other weekend

  • No holiday requirement no on-call no floating
