Requirements
Key Responsibilities
Perform thorough psychiatric assessments and mental status evaluations for residents in nursing homes.
Establish accurate diagnoses and design personalized treatment plans tailored to each patients needs.
Provide ongoing medication management making timely adjustments based on clinical progress.
Work collaboratively with physicians psychologists social workers nursing staff and other healthcare professionals to coordinate comprehensive care.
Deliver supportive counseling and psychotherapy to patients and their families.
Educate residents and caregivers about psychiatric conditions treatment options and practical self-management strategies.
Maintain precise and compliant documentation within the electronic health record (EHR) system.
Participate in case reviews interdisciplinary meetings and quality-improvement initiatives that strengthen clinical outcomes.
Qualifications
Active New York State Nurse Practitioner license with certification as a PsychiatricMental Health Nurse Practitioner (PMHNP).
Ability to work independently while managing a consistent caseload.
Strong diagnostic abilities and familiarity with psychopharmacology and evidence-based treatment approaches.
Excellent communication skills and the ability to build rapport with patients families and care teams.
Proficiency in electronic health records and basic computer applications.
A compassionate patient-focused approach to behavioral healthcare.
