Job Title: Social Worker (Hospice)
Location: Woodbury NJ
Type: Contract
Duration: 53 Weeks
Position Overview:
We are seeking a dedicated and compassionate Hospice Social Worker to join our interdisciplinary care team in providing high-quality end-of-life support for residents in a long-term care setting. The ideal candidate will provide emotional social and spiritual support to terminally ill residents and their families ensuring comfort and dignity throughout the hospice care process.
Key Responsibilities:
Deliver compassionate hospice and palliative care services in compliance with federal and state regulations.
Provide emotional counseling grief support and resource guidance to residents and families during the end-of-life journey.
Participate in weekly interdisciplinary team (IDT) meetings to discuss patient care plans and provide recommendations.
Collaborate with nursing pastoral care and clinical staff to meet each residents holistic care needs.
Offer follow-up bereavement support to families for up to one year following a residents passing.
Assist in coordinating hospice-related services including nursing home health aide and pastoral care support.
Participate in quarterly in-service training and contribute to the development of staff education initiatives.
Maintain accurate timely documentation of resident interactions and care planning.
Uphold confidentiality professionalism and sensitivity in all interactions with residents and families.
Qualifications:
Active Licensed Social Worker (LSW or LMSW) in the State of New Jersey (LCSW preferred).
Experience in hospice palliative or long-term care settings preferred.
Strong ability to support terminally ill patients and families with empathy respect and cultural sensitivity.
Excellent communication counseling and documentation skills.
Availability to participate in hospice support as needed including occasional weekends or holidays.
Must maintain all required certifications and licenses throughout the duration of the contract.
