Job Title: RN Hospice
Location: Woodbury NJ
Type: Contract
Duration: 53 Weeks
Position Overview:
We are seeking an experienced and compassionate Registered Nurse (Hospice) to provide high-quality end-of-life care to residents in a long-term care environment. The ideal candidate will demonstrate exceptional empathy strong clinical judgment and the ability to collaborate closely with an interdisciplinary care team to support both residents and their families through the hospice process.
Key Responsibilities:
Provide comprehensive hospice care in accordance with federal and state regulations.
Deliver compassionate physical emotional and spiritual support to terminally ill residents and their families.
Participate in weekly interdisciplinary team (IDT) meetings to review patient concerns and care plans.
Ensure that care services meet residents holistic needs social emotional and spiritual.
Coordinate with other hospice professionals including licensed social workers home health aides and pastoral care providers.
Maintain communication and follow-up with families for up to one year after a residents passing.
Participate in quarterly staff education sessions and ongoing in-service training related to hospice care standards.
Document and maintain accurate clinical records in compliance with regulatory guidelines.
Collaborate with facility leadership to ensure the continuity and quality of hospice services.
Requirements:
Active Registered Nurse (RN) license in the State of New Jersey.
Availability to support hospice services 24/7 year-round (rotational or as needed).
Experience providing hospice or palliative care in long-term care or home health settings.
Strong ability to support terminally ill residents and their families with compassion and professionalism.
Familiarity with hospice protocols state regulations and end-of-life care documentation.
Excellent communication organization and collaboration skills.
Must maintain all required certifications and licensure throughout the duration of the contract.
