Clinical Pharmacist Acute Care / Teaching Hospital (Contract California Region)
Overview
A leading acute care teaching hospital is seeking a skilled Clinical Pharmacist to support its inpatient clinical and operational pharmacy services. This role involves direct collaboration with medical teams therapeutic monitoring and ensuring safe and effective medication use across diverse patient populations. Ideal candidates will have strong acute care experience and comfort working in a high-acuity teaching environment. This is a 13-week contract offering competitive weekly compensation.
Key Responsibilities
- Verify medication orders for accuracy safety and compliance with clinical guidelines
- Provide pharmacotherapy expertise including dose adjustments and therapeutic drug monitoring
- Participate in interdisciplinary rounds to support evidence-based medication management
- Oversee medication preparation and dispensing in accordance with regulatory and institutional standards
- Conduct medication regimen reviews for interactions contraindications and optimization opportunities
- Support antimicrobial stewardship and other clinical pharmacy initiatives
- Perform medication reconciliation during transitions of care
- Document clinical interventions and medication-related activities in the electronic health record
- Respond to drug information inquiries from healthcare providers
- Ensure compliance with controlled substance handling and inventory protocols
- Contribute to departmental quality safety and performance improvement efforts
Qualifications
- Active pharmacist license in the state of practice (or eligibility to obtain)
- Recent experience in an acute care or teaching hospital setting strongly preferred
- Strong clinical knowledge across adult inpatient care populations
- Ability to collaborate effectively in a multidisciplinary academic environment
- Demonstrated proficiency with electronic health records and hospital pharmacy systems
- Professional references and completion of required competency assessments
Work Environment & Schedule
