The Department of Pediatrics Division of Complex Care at the Medical College of Wisconsin (MCW) and Childrens Wisconsin (CW) seeks a board certified / board eligible physician to join our expanding Complex Care Division/Department. We are comprised of nationally and internationally renowned dedicated group of faculty and staff with a strong commitment to clinical care.

Position Highlights:

  • Direct Care and Medical Care Coordination: Use care coordination as a strategy to assist in implementing the plan of care for special needs patients in collaboration with the family nurses and physicians in the Complex Care Program primary care subspecialists and other providers. Examine and assess patients prescribe medication and prescribe changes in treatment. Synthesize and evaluate the patient/family responses to therapy; make independent judgments; offer input on discharge needs; evaluate effectiveness of discharge planning as a transition to home and adaptation to illness. Provide expert advice which incorporates the broader aspects of patients/families response to illness. Seek opportunities for providing information and initiating consultation. Responsibilities include inpatient rounds outpatient clinic visits home visits occasionally writing and updating comprehensive clinical summaries in compliance with program procedure documentation and billing procedures.
  • Advocacy: Advocate for children with special health care needs and their families in healthcare public policy with community organizations and in other forums.
  • Education and Outreach: Present educational programs and conferences. Communicate content for formal and informal teaching and act as resource for patients/families staff and community. Participate in Complex Care research and quality improvement efforts.
  • Evidence Based Practice: Apply current research findings in own practice. Introduce current research into clinical collaboaration with the Complex Care team develop and implement research proposals that fit with the programs mission and goals. Contribute to the body of knowledge of nursing and related disciplines
  • Program Development: Contribute to program development efforts particularly clinical care improvement and activities in collaboration with the Complex Care team. Collect patient-related data to comply with program requirements and needs. Participate in professional improvement activities including professional organization membership.
  • Clinical .9% and Responsibilities include inpatient rounds outpatient clinic visits home visits occasionally writing and updating comprehensive clinical summaries in compliance with program procedure documentation and billing procedures

  • Team dynamic: program consists of 5 faulty 11 APPs 11 RN case managers 11 Care coordinatiors 2 social workers and 2 administive asssitance (faculty/APP ratio other providers/clinicians they may interact for clinical support etc.)
  • Required: Milwaukee Campus
  • Performs other duties as assigned by Chair or Chair-Designate.

Schedule:

  • 12-13 inpatient weeks per year; 60 continuity patients
  • second call responsibilities while inpatient
  • Salaried (Exempt)

Individuals should possess the ability to combine the clinical experience and proficiency needed to care for children with medical complexity and contribute to developing and evaluating new models of care for the complex care population.

Candidates should be board certified or eligible in pediatrics. Experience in caring for children with medical complexity is an asset.

Participation in outcomes-based research is expected along with the potential for further protected time for health services research and participation in leadership and other academic opportunities are available based on individual interests and qualifications.

The Complex Care Program provides care coordination and medical co-management for a growing number of children with medical complexity and high resource use. Our program is actively engaged in all missions of the Medical College of Wisconsin and Childrens Wisconsin including clinical care education academic scholarship and community service.

Childrens Wisconsin and Childrens Specialty Group
Childrens Wisconsin is an independent health care system dedicated solely to the health and well-being of children. The hospital with locations in Milwaukee and Northeast Wisconsin is recognized as one of the leading pediatric health care centers in the United States. Childrens Wisconsin provides primary care urgent care emergency care specialty care mental and behavioral health services community health services foster and adoption services child advocacy services and health insurance plans. Each year the Childrens Wisconsin team has more than 6.5 million touchpoints with kids and families through medical care advocacy research and Northeast Wisconsin Childrens Wisconsin is expanding its services to provide increased access to outstanding pediatric subspecialty care.
