We are seeking an experienced Hospice Clinical Manager to lead our hospice care team in Watertown. This team plays a vital role in providing meaningful support to terminally ill patients and their families ensuring comfort dignity and high-quality clinical care.
Key Responsibilities
- Lead and mentor a team of Home Hospice Case Managers fostering a collaborative and supportive work environment.
- Oversee and coordinate patient care services ensuring seamless communication between all providers and disciplines.
- Manage hospice staff operations including recruitment hiring onboarding training performance development and ongoing support.
- Supervise patient assessments and ensure care plans align with hospice standards and patient needs.
- Collaborate with caregivers insurance agencies healthcare partners and community resources to ensure comprehensive care delivery.
- Educate patients families and team members on the benefits and philosophy of palliative and hospice care in the home setting.
Qualifications
- Bachelor of Science in Nursing (BSN) required.
- Active RN license.
- Minimum of 3 years of hospice or oncology nursing experience.
- At least 1 year of leadership supervisory or managerial experience.
