POSITION SUMMARY Under the direction of the Community Health Outreach Manager the Community Health Representative (CHR) assists clinic clients and/or Tribal members with general medical and social service needs including patient advocacy care coordination and accompanying to medical appointments. The CHR acts as a liaison between Health and Human Services Department (HHSD) and community. ESSENTIAL FUNCTIONS The Nottawaseppi Huron Band of the Potawatomi reserves the right to change amend add delete and otherwise assign any and all duties responsibilities and positions titles as it deems necessary to meet the needs of the government. - Provides transportation accompanies clients to medical appointments as part of their care plan and performs patient advocacy as needed.
- Assists with home monitoring of clients and providing direct patient care within scope and capacity as identified in the client care plan i.e. basic screenings and basic needs as directed by the Provider and or Clinic Care Manager - RN.
- Assesses homes for safety and assists with service coordination to provide safety in the home.
- Assists with pickup and delivery of pharmacy orders and or other medical related items per policy.
- Provides education on health services and programs offered by CHR and HHSD.
- Motivates and encourages clients to obtain care and other services by facilitating community-clinic linkages through internal and external resource coordination and referral.
- Supports self-management of disease prevention and management of both acute and chronic disease conditions.
- Participates in monthly case conference meetings and gives feedback and updates as necessary.
- Provides social and emotional support activities to clients as requested or indicated by individual care plans or group needs.
- Coordinates with Chore Provider and assists with homemaker services and grocery shopping as needed.
- Assists in registering clients for assistance programs related to their needs.
- Develops and maintains relationships with both internal and external community resources and partners including but not limited to: social services housing food pantries elder care resources VA senior services etc.
- Works with other NHBP departments for programming and events.
- Visits clients and/or Tribal Members in hospitals nursing homes adult foster homes and home-bound situations to support care coordination activities as requested.
- Provides culturally relevant health education health coaching and health screenings individually during home visits and at community events.
- Enters patient data into Electronic Health Record as necessary for documentation.
- Assists with grant programs and objectives as directed and submits reports as necessary.

- Ability to successfully complete the Community Health Representative basic training program offered by IHS and the MiCHWA certification as soon as training is available.
- Ability to obtain First Aid and CPR as soon as training is available.
- Must maintain a valid drivers license with good driving record; GSA certifiable and must be able to travel.
- Must be able to successfully pass an NHBP background check and drug screen.
- Ability to maintain strict confidentiality including a demonstrated understanding and application of HIPAA.
- Ability to understand gain knowledge and appreciate the differences with the Native American culture and customs.
- Ability to work cooperatively as part of a multi-disciplinary health care team.
- Ability to work with constant interruptions and adjust schedule as needed or required.
- Computer proficiency including word processing spreadsheets email and internet
- Proven organizational and time-management skills; self-starter and self-directed; effective communication skills both verbal and written.
OTHER SKILLS AND ABILITIES - A willingness to initiate contacts with new people.
- Ability to interpret basic health information.
- Demonstrated ability to exercise judgment use discretion and use problem solving strategies.
- Ability to learn interpret and apply the NHBP and IHS regulations ordinances and Resolutions.
- Ability to maintain a good working relationship with Tribal Members co-workers NHBP Departments and the general public.
- Ability to recognize and work within scope of practice and authority.
PREFERRED QUALIFICATIONS - Advanced licensing or certification i.e. Licensed Practical Nursing
- Familiarity with Indian Health Services and other applicable federal programs.
