Job Summary

Navitas Healthcare LLC is seeking Licensed Master Social Worker (LMSW)for an exciting job in New York NY.

Responsibilities

Conduct comprehensive psychosocial assessments including discharge planning and crisis intervention.
Collaborate with physicians nurses and case management teams to ensure coordinated safe and timely patient transitions.
Provide counseling advocacy resource referrals and support to patients and families.
Develop individualized care plans addressing medical emotional social and environmental factors.
Maintain accurate and timely documentation in accordance with organizational and regulatory requirements.
Participate in interdisciplinary team meetings and contribute to ongoing care planning.
Uphold ethical standards patient confidentiality (HIPAA) and licensure regulations.

Required Skills & Experience

Minimum 1 year of LMSW experience; hospital or inpatient experience strongly preferred.
Strong clinical interpersonal and crisis intervention skills.
Ability to manage complex cases in a fast-paced clinical setting.

Education & Licensure

Masters Degree in Social Work (MSW) required.
Active Licensed Master Social Worker (LMSW) credential issued by NYSED (primary source verification required).
