Responsibilities:
- Accept and treat patients within your designated geographic area
- Provide 1:1 treatment
- Evaluate patients and develop individualized treatment plans based on physician orders and patient needs
- Deliver care that aligns with professional standards agency policies and the prescribed plan of care
- Educate family members home health aides and other caregivers on proper therapy techniques
- Plan and carry out occupational therapy programs to maintain improve or restore physical function relieve pain and prevent occupational dysfunction in patients.
- Perform and document an initial exam evaluating the data to identify problems and determine a diagnosis prior to intervention.
- Provide timely and accurate documentation for each visit
- Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit
- Must be willing to treat geriatric patients.
Qualifications :
- Occupational Therapist Program Graduate
- Licensed Occupational Therapist in the state of New York or eligible.
- Mandatory Certifications: First Aid AED & CPR Certifications.
- Home health experience is preferred but not required - We will provide you with a mentor to help with documentation and compliance.
- Ability to travel to patients homes within the assigned geographic territory
- Strong communication skills between patients support staff and management including timely responses via text email phone etc.
