Job Description
The Hospice Social Worker provides medical social services to patients receiving end of life services. The Hospice Social Worker provides assessments and a plan of care considering the unique bio-psychosocial needs of the patient and family. The Hospice Social Worker provides education support individual and family counseling linkage to community resources/services and ongoing collaboration with the IDG and community health providers
Additional Job Description
Education: Masters degree in a social work or social welfare program accredited by the Council on Social Work Education. Credentials: MSW licensure by the South Carolina Board of Social Work Examiners. Certification in case management preferred.
