CMHA is looking to hire a Home Health Aide for our Older Adult Care Team. This position will be located at 233 Main St. New Britain CT 06051. Monday-Friday 8:00AM - 4:00PM.
Compensation Range:The salary range for this position starts at $19.00 per hour.
Compensation for each position is determined not only by years of relevant experience education and skills but also by maintaining internal pay equity. Position Summary:Provides non-medical support to a caseload of older adult clients (55 plus) to enable clients to stay in their homes. The Home Health Aid provides light housekeeping laundry services shopping for food and/or other household items as needed light meal preparation and running errands. Partners with Registered Nurse and Clinical Case Manager to provide wrap around services by observing physical and mental health condition. Documents services and other client health-related data in EMR.
Essential Responsibilities:
- Conducts home visits and supports clients with light housekeeping laundry services shopping for food and other household items
- Provides companionship and personal engagement with appropriate and professional boundaries
- Supports with light meal preparation and running errands
- Helps to remind clients of taking medications and/or remind them to keep record of medications
- Assist clients in getting to and from medical appointments when needed
- Works closely with RN and Clinical Case Manager of the Older Adult Care Team on providing support to client so they can stay in the least restrictive level of care
- Works with team to provide education and support to family and loved ones of the client on topics related to aging memory elder services ascents out of home placements and other items as needed
- Collaborates and coordinates care with all primary and specialty care providers to arrange for care as dictated by the treatment plan.

Requirements:
- High School Diploma
- Must have valid Drivers License.
- Will become certified in MANDT First Aid and CPR.
- Participate in trainings on an annual basis as assigned.
- 3-5 years of working with older adults (55 plus) in home setting; some knowledge of serious mental health illnesses preferred
