Job Title: Full-Time Physical Therapist (PT) - Home Health (w/ $1.5k Sign-On Bonus)
Location: San Marcos CA (Territory: Anywhere in North County San Diego)
Position Type: Full-Time Monday - Friday Day Shift with weekend rotation
Compensation: $55 - $65 per hour / 40 hours per week
Job Overview
We are seeking a licensed Physical Therapist to deliver one-on-one in-home care across North County San Diego and neighboring communities. The PT will develop and implement tailored treatment plans educate patients and caregivers and collaborate with a multidisciplinary team to restore mobility and independence.
Responsibilities

Conduct comprehensive patient evaluations in home settings and develop individualized PT treatment plans.

Deliver therapeutic exercises manual therapy and functional mobility training to improve strength balance and independence.

Educate patients and caregivers on safety home-exercise programs and long-term self-management strategies.

Coordinate care with nurses OTs social workers and physicians to ensure seamless interdisciplinary support.

Document patient progress outcomes and plan updates in the electronic medical record within required timeframes.

Maintain HIPAA compliance safety standards and equipment sanitation on every visit.
Qualifications

Active California Physical Therapist license (Doctorate or Masters preferred)

Current CPR/BLS certification

Reliable transportation with auto insurance and valid CA drivers license

Minimum 1 year of clinical PT experience

Authorization to work in the U.S.

Open to new graduates

Strong clinical reasoning communication and patient-education skills

Ability to work independently manage a dynamic caseload and adapt to changing patient needs
Ideal Candidate Profile
A self-motivated licensed California PT preferably with at least one year of home-health experience. Excels at independent decision-making patient education and EMR documentation. Thrives in fast-paced travel-intensive roles and values interdisciplinary teamwork and continuous learning.
Day-to-Day Activities

Perform evaluations and deliver one-on-one therapy sessions focusing on strength balance and mobility.

Educate patients and caregivers on home exercises.

Complete EMR charting between visits and coordinate care with nurses and OTs.

Adjust treatment plans based on real-time patient feedback.
Team & Collaboration

Reports to the Clinical Manager of Home Health Services with dotted-line collaboration to the Regional Director of Therapy.

Join a multidisciplinary home-health team of 20 clinicians including PTs OTs nurses social workers and administrative support.

Collaborate daily with a PT mentor and connect regularly with case managers and rehab aides.
