Geriatric Support Services Coordinator
The Senior Care Options (SCO) Geriatric Support Services Coordinator (GSSC) is responsible for assessing SCO-enrolled consumers for need and eligibility for in-home services recommending service plans providing information on available resources and coordinating in-home/community services. The GSSC maintains member records and works in close collaboration with the SCO coordinators and nurses to provide comprehensive service planning for elders in the community setting. Schedule: This is a Local Union 509 full-time non-exempt (35 hour/week) position. We offer 4-day work week and hybrid (in-person & remote required 1-2 days in office) work schedule (once training is complete.) No nights or weekends.
Compensation: Starting salary range is $50872.46 - $54804.09. Being bilingual is not a requirement for this role but we recognize it as an asset and offer an additional 6% bilingual designation for Spanish Haitian Creole Russian Vietnamese Cantonese or Mandarin.

Job Activities
Job activities of the position are below. Additional duties may be assigned as required.
Top Key Activities:
- Be knowledgeable regarding multicultural community services and programs.
- Conduct initial and on-going in-home assessment of consumers health and functional status.
- Participate in development of consumers care plans including determining appropriateness and need for in-home/community-based services.
- Participate as part of consumers primary care team including attending team meetings trainings and joint home visits.
- Arrange coordinate and oversee in-home and community-based services for consumers.
- Monitor consumers health status in conjunction with care team to assess for effectiveness of current services need for additional services and/or need for long-term placement.
- Assist consumers with MassHealth redeterminations and other benefits paperwork as requested and appropriate.
Other Activities:
- Maintain consumer records in accordance with standards.
- Compile and submit required reports.
- Assure consumers are connected to appropriate multicultural community programs and services that address social determinants of health and wellbeing.
- Participate in interdisciplinary/collaborative model of case management
Qualifications:
- A Bachelors degree in Social Work or a related field
- Strong interviewing and assessment skills.
- Strong interpersonal and organizational skills.
- Excellent interpersonal skills with demonstrated ability to communicate across all levels including staff family and external partners
- Ability to maintain confidentiality while working with sensitive information or in sensitive situations.
- Highly resourceful team player with the ability to work effectively independently or as a team.
- Demonstrates a willingness to take on new tasks with a general attitude that no task is too small impossible or cannot be improved
- Expertise in the following platforms: Microsoft Office 365 (Word Outlook Teams Excel and PowerPoint) Adobe Pro Zoom and other web-based applications.
- Private Transportation and valid drivers license.
Preferred Qualifications:
- Social Work license.
- Knowledge of and experience working with older adults in a community-based setting.
- Experience with complex case management (medical or behavioral).
- Bilingual (Spanish Haitian Creole Russian Vietnamese Cantonese or Mandarin).
Physical Requirements of the Job:
- Ability to lift push pull and carry objects weighing up to 25 pounds
- Ability to travel to the community to perform outreach
- Work requires regular standing stooping and bending
