About the role
The DiabetesEducator works in partnership with clients and their families/carers to provide health education therapeutic management and support services as required for diabetes prevention and/or management within a chronic condition self-management framework. The DiabetesEducators work within a social model of health and with the broader team to address identified problems with clients.
Classified under the Nurses and Midwives (Victorian Public Sector) (Single Interest Employers) Enterprise Agreement 2024-2028.
About you
We are seeking a Credentialed Diabetes Educator or working towards status through the ADEA credentials program. You will have excellent communication negotiation organisational and time management skills. You will also have a strong understanding and commitment to the principles of community health and have the ability to work autonomously as well as part of a multidisciplinary team. Experience with working with people from a culturally and linguistically diverse or refugee background is desirable.
