Position Summary

The Registered Nurse (Home Health) provides comprehensive patient care in a home setting coordinating with physicians and interdisciplinary teams to ensure high-quality outcomes and continuity of care. This position supports patients throughout Western Pennsylvania and offers flexibility autonomy and a structured onboarding program.

Key Responsibilities

Deliver skilled nursing care to patients in their homes in accordance with the plan of care.
Perform initial and ongoing assessments of patient needs condition and environment.
Administer medications treatments and wound care per physician orders.
Educate patients and family members on disease management medications and care plans.
Collaborate with physicians therapists and other clinicians to optimize patient outcomes.
Document all care activities accurately and promptly using electronic medical records.
Maintain compliance with federal state and organizational regulations.

Required Qualifications

Active Registered Nurse (RN) license in Pennsylvania
Associate degree or higher in Nursing
Minimum 1 year of RN experience (Home Health preferred)
AHA BLS required; AHA ACLS accepted
Valid drivers license reliable insured vehicle and ability to travel within assigned territory
Strong communication organization and time-management skills

Preferred Qualifications

Prior Home Health or Hospice experience
Proficiency with OASIS documentation
Experience in wound care IV therapy or post-surgical management
