About the Role
As a Physical Therapist in Union Grove you will help residents improve mobility regain function and maintain independence. Youll develop individualized treatment plans collaborate with a skilled interdisciplinary team and deliver meaningful care that changes lives.
Key Responsibilities
Conduct comprehensive assessments of residents physical function and mobility
Develop and implement individualized treatment plans
Provide skilled physical therapy interventions
Educate patients and caregivers on mobility strategies and fall prevention
Recommend appropriate assistive devices and modifications
Maintain accurate timely clinical documentation
Collaborate with nursing and therapy colleagues to ensure holistic care
Qualifications
Graduate of an accredited program with a bachelors masters or doctoral degree in Physical Therapy
Licensed or eligible for licensure in the State of Wisconsin
Strong clinical communication and organizational skills
A passion for working with older adults in a skilled nursing or rehabilitation setting
