Description

In an environment of continuous quality improvement the COPD Navigator will execute all necessary action to reduce/eliminate exacerbations and slow COPD related respiratory clinical decline in patients. Exhibits the MHS Standards of Excellence and exercises strict confidentiality at all times.

Job Functions:

  1. Assists in development and implementation of quality improvement processes for the management of COPD patients across the care continuum
  2. Identifies care gaps and facilitates closures
  3. Monitors and tracks all COPD readmissions
  4. Identifies and implements measures to reduce COPD readmissions
  5. Consults and collaborates with the multi-disciplinary team to coordinate patient plan of care
  6. Verify patients understanding and ability to properly use home medications
  7. Recommend medications and treatment pathways based on GOLD guidelines
  8. Perform patient needs assessment to identify and address barriers to care
  9. Participate in the development of standardized processes and procedures that support efficient and effective continuity of care practices
  10. Connects patients to key Pulmonary services
  11. Educate patients family and providers on COPD care
  12. Provides self-management education and champions individual care plans
  13. Assumes all other duties and responsibilities as necessary.

Qualifications

Minimum Education/Experience Required:

  • Registered Respiratory Therapist Licensed by the state of Ohio with at least 7-years experience as a Respiratory Therapist required.
  • Completion of the AARC Pulmonary Disease Educator Course within 3 months of hire required.

Special Knowledge Skills Training:

  • Thorough understanding of GOLD Guidelines.

Compensation Details: Education experience and tenure may be considered along with internal equity when job offers are extended.
