Shift: Day Shift 5x8 Hours (8:00 AM 4:00 PM)
Hours per Week: 40
Pay Rate: $51.00 $53.00/hr

Position Overview:

Navitas Healthcare LLC is seeking Physical Therapist (SNF)for an exciting job in Syracuse NY.

Responsibilities:

Evaluate plan and implement individualized treatment programs following established physical therapy standards.
Develop patient-specific goals in collaboration with the interdisciplinary healthcare team.
Accurately document patient progress evaluations and discharge summaries.
Participate in care conferences clinical rounds and interdisciplinary meetings.
Educate patients and families on exercise programs and home rehabilitation techniques.
Mentor new staff members and serve as a clinical resource when needed.
Contribute to program development and continuous improvement of therapy services.

Qualifications:

Education: Bachelors Degree (B.S.) or higher in Physical Therapy.
Licensure: Active or eligible New York State Physical Therapy License required.
Experience: Minimum 2 years of professional experience in physical therapy; skilled nursing or outpatient experience preferred.
Certifications: Current BLS certification preferred.
