Job Summary

This position provides direct patient care to hospice and home health patients in their home or place of residence. The CNA assists in providing a clean and safe environment. They work cooperatively with patients families and caregivers and share observations and changes with the RN care manager. They work under the supervision of a Registered Nurse.

Required Qualifications:

  • High School Diploma or equivalent
  • Virginia State Approved CNA program Certification
  • Educational requirements per hospital certifications (Example: Stroke Orthopedic/Spine)
  • Current Virginia State CNA License
  • Basic Life Support - BLS

Preferred Qualifications:

  • Previous home health and hospice experience

Skills:

  • Mandatory Competencies required by all Augusta Health Employees annually
  • Transfer Training
  • Must be ablet to read write and verbally report clinical information to patients representatives and caregivers as well as other hospice team members
  • Demonstrates compassion for patients at end of life
  • Ability to work in patients homes in various conditions
  • Ability to work a flexible schedule
  • Ability to travel to and from patient homes in various weather conditions
  • 12 hours of additional in-service per year
