Job Title: Memory Center Coordinator
Location: Trinity Elms Assisted Living
Job Type: Full-Time On-Site
Schedule: Monday through Friday 8 hours per day
Pay Range: $18 - $20 per hour
Position Summary:
Trinity Elms Assisted Living is seeking a dedicated and skilled Memory Center Coordinator to manage the daily operations of our memory care center. The successful candidate will be responsible for upholding the highest standards of quality care ensuring compliance with all federal state and local regulations as well as facility policies and guidelines.
Key Responsibilities:
- Uphold the philosophy and objectives of the memory care neighborhood.
- Ensure sufficient staffing levels to meet resident needs.
- Complete nursing documentation accurately and on time.
- Attend community and facility meetings as required.
- Monitor supply levels and evaluate needs according to resident requirements.
- Take assignments as needed to ensure residents receive appropriate care during staff shortages.
- Prepare necessary reports as assigned.
- Assess and document the needs of newly admitted residents.
- Develop implement and review resident care plans in accordance with regulatory timelines.
- Supervise nursing staff in collaboration with the administrator including conducting evaluations providing coaching and administering disciplinary actions as necessary.
- Adhere to established safety protocols including proper lifting and transfer techniques fire and disaster procedures use of personal protective equipment and reporting exposure to hazardous materials or bloodborne pathogens.
- Participate in educational programs and complete required in-service training.
- Embody Trinity Elms mission vision values and commitment to customer service in all interactions.
- Perform additional duties as assigned.
Qualifications:
- High school diploma or equivalent.
- Must be 21 or older and have 6 months experience as a med tech
- Experience in long-term care particularly with individuals living with dementia.
- Current CPR certification.
- Familiarity with state and federal regulations governing adult care homes and specialized units.
- Proficiency in electronic medical records and basic computer skills.
- Medication technician certification or NC nursing license preferred.
Physical Requirements:
- Ability to move throughout the facility as needed.
- Capable of lifting up to 35 pounds and assisting with the transfer of individuals of varying weights following safe practices.
- May require extended periods of standing or walking sometimes while transporting objects.
- Occasional bending squatting or repeated reaching may be necessary.
