Responsible for acting as liaison between patients families caregivers and various communitybased organizations to ensure that the program consistently provides optimal care and service experiences that surpass expectations. As the designated patient liaison for the Destination Medicine Program coordinates health care services and the patient care experience for contracted Destination Medicine agreement patient referrals and for patient self-referrals for health services. Responsible for logistics including but not limited to travel accommodations language translation and concierge services.Job Duties:
