Responsibilities:
Deliver direct patient care including administering medications and treatments as prescribed.
Monitor patient conditions record observations and communicate changes to the healthcare team.
Assist with patient rehabilitation activities and support activities of daily living (ADLs).
Maintain patient safety and provide compassionate care to residents.
Adhere to all federal state and facility policies and procedures.
Qualifications:
Current LPN licensure in Vermont or eligible for licensure.
Minimum of 1 year of experience in long-term care skilled nursing or rehabilitation settings required.
Strong clinical skills and the ability to work independently and as part of a team.
Excellent communication and interpersonal skills.
Ability to adapt in a fast-paced healthcare environment.
